I. Enrollment and Eligibility

1.1 Eligibility Requirements

The membership program is available to individuals who satisfy Evolve Your Bod’s eligibility standards, as determined during the onboarding process specified in the General Terms.

1.2 How to Enroll

To join the membership program, participants must:

- Complete the standard onboarding process, which includes identity verification and submission of relevant medical history.

- Agree to the program’s terms, including payment obligations and authorization for recurring billing.

1.3 Participation Restrictions

- Membership is only available to individuals residing in states where Evolve Your Bod is authorized to operate.

- Participants must remain in compliance with all medical and health requirements established by Evolve Your Bod throughout their enrollment.

II. Subscription Pricing and Payment Structure

2.1 Subscription-Based Pricing

- Members commit to a specified service term (e.g., 3, 6, 9, or 12 months) at a discounted monthly rate, as outlined at the time of enrollment.

- Instead of paying the full amount upfront, participants are charged monthly, with the fee not dropping below the agreed-upon minimum rate.

- If a participant's treatment plan changes during their commitment term, monthly costs may increase based on the updated care plan.

- Subscription fees cover access to telehealth services, prescribed medications, and additional program benefits.

2.2 No Refund Policy

Payments made under the membership program are non-refundable, except in cases where a refund is required by law or explicitly outlined in these Terms.

2.3 Automatic Renewal

- Memberships automatically renew at the end of the term for another term of the same duration at the prevailing rate, unless canceled before the renewal date.

- Members will be notified at least 30 days in advance of their renewal date, including the updated rate and cancellation instructions.

- To prevent renewal, members must submit a cancellation request via the Evolve Your Bod member portal or by contacting customer support prior to the renewal date.

2.4 Pricing Adjustments

- Evolve Your Bod reserves the right to modify pricing for future renewals. Any changes will be communicated at least 30 days before the current term expires.

III. Included Membership Services

The specific services provided through the membership program depend on the selected plan:

GLP-1 Program

- Includes an initial consultation and ongoing telemedicine visits with healthcare providers.

- Prescriptions and shipment of compounded GLP-1 weight management medications.

- Available as a 6-month or 12-month plan.

Lite Program

- Provides a more limited selection of services.

- Available in 3-month, 6-month, or 12-month plans.

All participants receive access to the Evolve Your Bod member portal for progress tracking, support, and plan management.

IV. Cancellations and Termination

4.1 Member-Initiated Cancellation

- Members may cancel their participation mid-term, but they will still be responsible for the remaining payments through the end of their commitment period.

- If a healthcare provider determines that a participant is unable to continue due to a medical condition, cancellation may be allowed without further financial obligation. Documentation may be required.

- Members will retain full access to the Evolve Your Bod member portal, providers, and services until all scheduled payments have been made.

- Cancellation requests must be submitted via the Evolve Your Bod member portal or through customer support.

4.2 Cancellation by Evolve Your Bod

Evolve Your Bod may terminate a participant’s membership if they:

- Fail to comply with medical instructions.

- Do not meet payment obligations.

In such cases, the participant remains responsible for any outstanding payments through the end of their term.

4.3 Cancellation Due to Medication Shortages or Regulatory Changes

If a medication shortage or regulatory change prevents Evolve Your Bod from continuing services, the program may be discontinued. Participants will be notified and may be offered an alternative product or service.

4.4 Effect of Termination

- If the program is discontinued, unused portions of the term will not be refunded unless specifically stated otherwise.

- Members whose participation is terminated for reasons unrelated to payment obligations will no longer have access to the Evolve Your Bod member portal, providers, or services.

V. Payment and Billing

5.1 Accepted Payment Methods

- Payments must be made using a valid credit or debit card that is authorized for recurring charges.

5.2 Authorization for Recurring Payments

- By enrolling, members authorize Evolve Your Bod to automatically charge their stored payment method for their membership fees.

5.3 Handling Failed Payments

- If a payment is unsuccessful, Evolve Your Bod may temporarily suspend access to services until payment is resolved.

5.4 Adjustments to Pricing

- Future pricing changes will be communicated at least 30 days before renewal, allowing members to cancel before the new rate takes effect.

VI. Program Limitations

6.1 Availability of Medications

- Evolve Your Bod does not guarantee the ongoing availability of any specific medication included in the membership program.

- If a medication is unavailable due to shortages or legal restrictions, alternative options may be offered, or the program may be terminated.

6.2 Medical Necessity

- Eligibility for continued participation and access to prescribed medications is subject to determination by Evolve Your Bod’s healthcare providers based on medical necessity.

VII. Dispute Resolution for Payments and Services

- Any disputes regarding billing or services must be submitted in writing to Evolve Your Bod’s customer support team within 30 days of the disputed charge.

VIII. Program Modifications and Discontinuation

- Evolve Your Bod reserves the right to modify, suspend, or terminate the program at any time.

- If the program is discontinued, members may receive prorated refunds for unused services unless prohibited by law.

IX. Relationship to General Terms

- Unless explicitly stated otherwise, the General Terms of Service apply to all aspects of participation in the membership program.

- In cases where a conflict arises between these membership terms and the General Terms, the membership terms will govern matters specifically related to the program.
